I think they want to make it more like something shocking and unexpected. Well for some especially young people it may be gross thing, but I think the main clue was that what I said first. And anyway they say at the end that teenage pregnancy is also shocking and unexpected.
But I'm wondering if this Advert will help even a bit... When did such shocking or even normal social adrevts changed the world? I don't remember... People always think, Ooh it won't happen to me... yeah
