What you see in a porn film is the result of several hours of work. Scenes are shot repeatedly. Special effects are employed (like fake cum). Creative camera angles are often employed to suggest actions that are not really happening. Then all the various shots are editted together to make it appear that you are watching continuos action.
While it's true that actors can train themselves to ignore normal sexual responses, most of what you see in a porn film is scripted, doctored and creatively editted.

I knew a guy in college who recorded the audio for some porn films. He watched the raw footage and recorded fake moaning and heavy breathing on a cassette recorder. The film maker would dub his fake sounds into the finished movie. My friend never saw the actual actors, he recorded his part in his dorm room. He got about $10/movie for his trouble.
