And I'll be honest: I don't think you got "it". You just have to let her know - listen this is who I am. If you don't like me being too nice, then you don't like ME. I'm going to be me no matter what though. TAKE IT OR LEAVE IT.
As far as the whole confidence/take control thing - like I said you just don't have it. It's no reason to panic or despair - I just don't think it's something we can "teach" you - only life experience can help you there. | 
I doubt she is muscular if her doctor told her she is 20 lbs overweight.
