Aye, read that. Simply:

A. Tell her how you feel, she will be flattered by this for starters, though she may want to think things through.

B. "Hey (name), ummm would you be interested in us starting a relationship together?" Look her in the eyes. Good in that it shows some gall, and it aint beating about the bush.

C. Covert operatives, get some friends of yours to befriend her or something like that, and get them to poke around for any hints as to how she feels about you. Such as getting them to tease her that she wants to go out with you, then giving you the response. This is a truely spineless thing to do, though it can be fun, and you are guaranteed of success in knowing how she feels about you without embarassing yourself. Though bare in mind if she is intelligent she will see right through it.

Don't tell her you want to start a relationship with her until after you've dated for a while. YOu might not like her so much once you get to know her better. Just ask her if she wants to go do something sometime, and have some ideas ready so you are prepared if she says yes on the spot. Try to be very casual, as if you are not hanging on desperately for her response.
